When someone causes harm or failure, the instinct is to identify a culprit and extract punishment — public naming, demotion, exclusion. This response is so normalized that practitioners confuse it with genuine accountability. Meanwhile, the system fragments: people hide mistakes, escalate conflicts rather than resolving them, and build parallel trust networks to work around formal channels. In activist spaces, this manifests as call-outs that generate factions rather than change. In corporate contexts, it creates compliance theater where people report what they think leadership wants to hear. Government agencies develop institutional amnesia, each scandal followed by restructuring that leaves the root causes intact. Tech products deploy accountability as algorithmic punishment — banning users, closing accounts — without understanding what went wrong. Across all these domains, the commons deteriorates because the system cannot learn from failure. What persists is the exhaustion of people who fear accountability, even when they want to do better.

Pause, acknowledge this, and redesign: What repair would actually restore trust? What timeline is credible? Reset the process with that honesty.
